Supervised by: GAFC Program Director

Job Summary: The Group Adult Foster Care Case Manager depicts, supervises and Coordinates the provision of social services to participant of the GAFC program.

Responsibilities:

Assists a Registered Nurse in evaluation of potential members for the GAFC program by assessing the psychosocial needs of the client
Assists in admission process and orientation of the participants to the program
Completes psychosocial assessment on admission
Visits the GAFC member within three (3) days of the admission, while personal care assistant present and completes appropriate documentation
Alternates every other week with the Registered Nurse for the first four (4) weeks and the every other month (alternates every other month with the RN)
Works with the Registered Nurse as part of the multi-disciplinary team
Monitors psychosocial needs of the member
Documents appropriately and in a timely manner
Qualifications:

BA in social work (license not required)
Maintains confidentiality of patient and Agency information at all times
Three years of previous experience in working with disabled/elderly adults in a community setting; OR An equivalent degree and/or experience may be substituted for the educational requirement
Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the client, clients family, and the personnel of the agency.
Physical Requirements:

Hear and speak in a manner understood by most people
Stoop and bend
Travel to prospective clients residence
Climb stairs
Read 12 point or larger type
Special Requirements:

Must have valid state Driver License and reliable transportation with current automobile insurance
Must be willing to work at various hours
Must attend and participate in continuing educational programs as they relate to job Responsibilities
